​AFFILIATE MEMBERSHIP APPLICATION

    Membership Application Instructions

    Definitions 

    Regular Affiliate Members: Suppliers to investment casting manufacturers. Regular Affiliate Members vote on articles of incorporation and all matters and motions proposed at membership meetings, and for directors who are Regular members.

    Associate Members: Manufacturers of investment castings outside North America do not have voting rights nor may they serve on the Board of Directors, but may vote on special assessments brought before the general assembly. Conventional Members: Companies applying for membership with foundries operating on a single continent.

    Hybrid Members: Companies applying for membership for foundries operating on multiple continents.

    Conventional Membership Instructions

    If your company wishes to register for a conventional membership, either Regular Affiliate (North American) or Associate Affiliate (International), please complete the form as follows:
    1. Please provide requested company information on page 3. This data shall be used for customer referrals, the Institute’s Annual Buyer’s Guide and distribution of INCAST Magazine, which is complimentary with your membership.
    2. Dues calculation and payment information is addressed on page 4.
              a. Please indicate your company’s total sales to the investment casting industry in the left column of the Membership Fee Rate Table.                                b. If your company operates in North American, check the box in the Regular column of the Membership Fee Rate Table, otherwise check the                      box in the Associate column.
             c. Complete the Method of Payment grid (in $ U.S.). 3. Applications may be completed online or emailed to [email protected].

    Hybrid (Multi-Continent) Membership Instructions

        Should your company wish to apply for a hybrid membership, please complete this form as follows:
    1. If the applicant has operations located in North America, that operation will be designated as the main point of contact for dues related to                financial transactions. If no such North American operation exists, then the lead international operation will be designated as the main point of contact for this purpose.
                                a. All membership invoicing and dues will be made through the designated main point of contact.
                                b. The main point of contact shall also serve the purpose of liaison for all company operations located on the same continent.
                                c. The Member Data sheet on page 3 of this application must be completed for the main point of contact.
    2. Supplemental Member Data sheets can be found at the end of the application starting on page 5.
                            a. One data sheet must be completed on a continent-by-continent basis for the continents on which the applicant has a base of                                                  operations seeking membership.
    ​                        b. The point of contact designated on these data sheets shall also serve the purpose of liaison for all company operations located on the                                same continent.
                           c. There will be a $250 processing fee assessed for each additional continental membership.
    3. Dues calculation and payment information is addressed on page 4.
                          a. Please indicate your company’s total investment casting and tooling sales for all foundries that you wish to enroll as a member in the                                left column of the Membership Fee Rate Table.
    ​                     b. If your application includes North American foundries, check the box in the Regular column of the Membership Fee Rate Table,                                           otherwise check the box in the Associate column.
                         c. Add $250 for each supplemental Member Data sheet completed to the amount of dues indicates in the Membership Fee Rate Table. The                             sum of these is your annual membership dues.
    4. Hybrid Member facilities operating in North America will be designated as Regular Members, with all the rights and privileges afforded that           designation. Those on other continents will be designated as Associate Regular Members, with all the rights and privileges afforded that designation.
    5. Applications may be completed online or emailed to [email protected].

    At the time of application for membership, companies pay dues for one full year. At the next dues billing, the new member will be assessed for dues in full, less a prorated amount reflecting that period of the previous year during which the company was not an ​accepted member of the Institute. (The Institute’s membership year is Jan.1-Dec. 31). Dues costs are determined by the Board of Directors. Membership will be automatically renewed each year unless member requests cancellation in writing on or before Nov. 30 each year.

    IMPORTANT NOTE

    ​If any answers or statements are false or materially or substantially false or untrue on this application and accompanying documents, then such applicant will be barred from membership and any dues paid will be forfeited to the Institute. Applicant, by signing this application, swears that information contained on this and all other accompanying documents is true and correct.
